Report an Accident or Incident

Students, faculty, and staff should report any incident or accident whereby they and/or someone else is injured, or damage has occurred to University or personal property, while on AHU or AdventHealth premises. The Incident Report Form should be completed within 24 hours of the incident. Questions or concerns can be directed to AHU Security at (407) 353-4002. Remember, in the event of an emergency, dial 911.

Submit an Academic Early Alert

The Early Alert was created to provide follow-up for students identified by faculty as having academic difficulties or other problems that may impact student success. The main objective of this alert is to assist students with issues early in the trimester to increase the chance of success and retention by providing proactive interventions as part of a university-wide effort. Questions related to an Academic Early Alert Form submission can be directed to Elizabeth Hernandez, Director of Student Academic Support Services, at elizabeth.hernandez@ahu.edu.

File a Behavioral Intervention Team (BIT) Report 

The AdventHealth University Behaviorial Intervention Team (BIT) promotes a healthy and safe environment by facilitating collaboration across campus departments to support students in distress. The team assesses reports of concerning behavior to determine appropriate intervention to address the physical and social welfare of the campus community. BIT centralizes incident information to identify patterns of concerning behavior and coordinate early intervention.If you are aware that a student is experiencing challenges or difficulties that concern you, please report the concern using the BIT Reporting Form below. Report Sexual Misconduct/Sexual Harassment (Title IX Incident Report)

AdventHealth University is committed to providing a learning, working, and living environment that promotes personal integrity, civility, and mutual respect in an environment free of discrimination on the basis of sex.  The University considers Sex Discrimination in all its forms to be a serious offense. All University employees have a duty to file a report or complaint with the Title IX Coordinator or Deputy Title IX Coordinator when they believe or receive information indicating that a member of the University Community may have been subjected to conduct that constitutes prohibited Sex Discrimination.  Students are encouraged to file a report or complaint.  Report a Concern (Students)

Students who have specific concerns or complaints regarding their experience at AdventHealth University and are unsure of the applicable process or reporting mechanism may file their concern(s) with the Dean of Students using the form below. Completed Student Concern Forms will be reviewed and the Dean of Students will communicate with the student to gain an understanding of the matter at hand and determine how the concern should be addressed, in accordance with AHU policy and/or procedure. Report a Violation of Academic Misconduct

When an instructor becomes aware of alleged student academic misconduct, they must decide if the behavior warrants formal documentation through the Academic Misconduct Report Form (AMRF) by identifying the alleged misconduct violations and proposing course sanctions. Academic misconduct includes, but is not limited to: cheating on examinations, plagiarism, fabrication, forgery, obstruction, multiple submissions, complicity, and misconduct in research assignments, computer misuse, and misuse of intellectual property.
